As a Registered Nurse Case Manager your day may look like this!

• Visit our adult and geriatric hospice clients for assessments, admissions, and regular visits
• Work closely with an interdisciplinary team
• Weekly  IDT meetings

If you’re a Registered Nurse (RN) with the following skills, this could be your dream job!

• Registered Nurse (RN) with 1 or more years of experience as an RN.
• Hospice experience preferred but not required
• RN License in the state of NH and VT or a compact license that covers both states
